This 64-year-old female came to Dr. Piazza at his Austin plastic surgery office location with a ruptured right breast implant found on routine mammogram. She wanted to discuss revision breast augmentation surgery and possibly removing her current breast implants and replacing them with new implants. She had a breast augmentation over 25 years ago with silicone gel breast implants through a periareolar incision. She had noticed any other changes in her breasts. In moving forward with a surgical procedure, she wanted to know if a breast lift could be performed at the time of the revision breast augmentation. She was referred to Dr. Piazza by her primary care physician. Dr. Piazza and his nurse, Hayley, spent an hour with her during her appointment. Dr. Piazza explained the safety of silicone gel implants and recommended the use of her previous breast incision at the lower portion of the areola. He explained that by removing excess skin of the breast around the areola, he could achieve a lift of the areola centering its location over the breast mound to give a more youthful appearance to the breasts. In surgery, Dr. Piazza removed the old silicone gel implants, revised the submuscular pocket and placed new silicone gel implants while performing a circumareolar (periareolar) breast lift. She tolerated the procedure very well. She returned to normal activity by 4 weeks without pain, discomfort or limitations. She is most happy with the appearance of her breasts. Her breast enhancement surgery was an outpatient procedure in an ambulatory surgical hospital in Southwest Austin, Texas.
