The question: Can you improve under-eye hollows without filler?

The short answer: A true hollow needs volume; a cream cannot add it

What a cream can do: Improve thin, crepey, dull skin that makes a hollow look worse

What we stock: Pavise Precision Eye Lift, with EXO EYE as an alternative

How to use it: Morning and night, tapped from the inner corner outward

Why This Matters: Most people searching for an eye cream that works are dealing with two or three problems at once, and a cream only reaches some of them. Knowing which one you have saves money and sets honest expectations. Individual results vary.

Patients point at the same spot under the eye and describe it a dozen ways: hollow, dark, tired, crepey, sunken. Those are not the same problem. Some are about volume and bone, some are about skin, and some are about pigment or blood vessels showing through.

I perform eyelid surgery, and I also stock eye creams. Both are useful, for different things. This is a plain account of what causes the under-eye look you are trying to change, what filler and surgery actually do, and where a well-made eye cream fits in.

Key Takeaways

  • A deep tear trough is a volume problem. Filler or surgery changes it; an eye cream does not.
  • Thin, crepey, dull skin makes any hollow look deeper, and that is the part a good eye cream can improve.
  • Expect weeks, not days, and judge results in the same light you took your "before" photo in.

Can an Eye Cream Fix Under-Eye Hollows?

No eye cream can fill a true under-eye hollow, because a hollow is missing volume beneath the skin, not a skin-surface problem. What a cream can do is improve the thin, crepey, uneven skin over the hollow, which often makes it look shallower and less shadowed. For a deep tear trough, the options that change the shape are filler, fat repositioning or eyelid surgery.

The useful question is not "does it work" but "which part of my under-eye is the problem."

What Is Actually Going On Under Your Eyes

A widely cited review in the Journal of Clinical and Aesthetic Dermatology sorts under-eye darkness into types, and they respond to different treatments. The authors are candid that there is very little scientific data on the condition and that most people have more than one type.

Four Things That Make Under-Eyes Look Tired

Structural hollow

Shadow from the tear trough: loss of fat and thinning skin over the orbital rim ligaments.

Cream helps? Only the skin over it

Thin, crepey skin

Thin skin over the orbicularis muscle lets vessels show through and creases easily.

Cream helps? ✓ Yes, gradually

Pigment

A brown tone from extra melanin, often made worse by rubbing and sun.

Cream helps? ✓ Partly, with SPF

Puffiness and bags

Fluid, or fat pushing forward. Fat that has moved does not move back with a cream.

Cream helps? Fluid briefly; fat no

Categories adapted from Sarkar et al., J Clin Aesthet Dermatol, 2016. "Cream helps?" reflects Dr. Piazza's clinical view.

The hollow itself. The tear trough is the groove running down and out from the inner corner of the eye. With age it deepens, which the review attributes to loss of fat and thinning of the skin over the ligaments of the orbital rim. Overhead light then casts a shadow into it. That is why under-eyes look worse in a bathroom mirror than in daylight.

The skin over it. Lower-lid skin is thin and translucent. When it loses density it creases into fine, papery lines and lets the blue-purple of the muscle and vessels show through. This is the crepey look, and it often comes on faster after weight loss, which we cover in our piece on crepey skin after GLP-1 weight loss.

Bags. The lower lid has fat pads that sit behind a thin membrane. Over time they push forward, and the bulge above the groove makes the hollow beneath look deeper still.

What Filler and Surgery Do That a Cream Cannot

If the main problem is structure, the honest answer is a structural treatment.

Filler. Hyaluronic acid filler adds volume to the groove. Two products are now FDA-approved specifically for under-eye hollows in adults over 21. AbbVie reported an 83.1% responder rate for Juvéderm Volbella XC against 15.6% in untreated controls, and Galderma reported that 87% of Restylane Eyelight subjects improved by at least one grade at three months. The trade-offs are cost, upkeep and the need for an injector who knows this area well.

Eyelid surgery. Blepharoplasty addresses what filler cannot: excess and sagging lid skin, fatty deposits and bags. In the lower lid we can remove or reposition fat to smooth the junction between lid and cheek. It is the most definitive option and has real recovery time.

What an Eye Cream Can Realistically Do

Here is where a good topical earns its place. If your hollows are mild, or you are not ready for a needle or a scalpel, improving the skin over the groove makes a visible difference to how the area reads. Denser, smoother skin reflects light more evenly and lets less of the purple tone through. Crepey lines soften. The shadow is still there, but it is less dramatic.

The same holds after filler or surgery. Neither one improves the quality of the skin itself, and patients who look after the skin tend to be happier with the overall result.

Three rules for judging any eye cream: give it at least four to six weeks, take a "before" photo in daylight from the same angle, and be suspicious of anything that claims to replace filler.

Why We Chose Pavise Precision Eye Lift

Pavise Precision Eye Lift is a concentrated eye-contour treatment built on the brand's DiamondCore technology and what Pavise calls a recombinant proteome matrix — signalling peptides, ceramides and sodium hyaluronate. It is aimed at density, crepiness and brightness, which are the right targets for this skin.

On the evidence, Pavise documents photographic before-and-after results at four weeks, and separately reports instrumental measurements at six weeks. On its own site, Pavise also states that 97% of users noticed firmer skin around the eyes in an eight-week third-party study. That is self-assessment, and the sample size is not published, so treat it as a signal, not proof.

We also tested the line on our own team. Kari, our medical aesthetician, used the full Pavise routine for three months through an Austin summer. Her photo below is honest about what it is: one person, the whole line rather than the eye product alone, and not a clinical trial. You can read the rest in our team test.

How to use it: after cleansing, morning and night, use about a grain of rice per eye. Tap it along the orbital bone from the inner corner outward; do not drag the skin. In the morning, sunscreen goes over it. If the area stings or waters, use less.

"When someone asks me for an eye cream that works, the first thing I do is look at them under good light and tell them which part of the problem a cream can reach. If it is the skin, a well-formulated topical used consistently is worth doing. If it is fat or bone, I would rather say so than sell them a jar." — Dr. Rocco Piazza, MD, FACS

The Alternative: EXO EYE

Some patients prefer an exosome-based formula. EXO EYE from Exoceuticals ($155) combines eXo3 exosomes, NAD+ and peptides, and is aimed at crepey texture, dark circles and the look of hollowing. The brand does not publish clinical results on our product page, so we present it as an option for people who want that ingredient approach, not as the stronger choice. Use one eye treatment, not both.

What We Stock

  • Pavise Precision Eye Lift ($128, 15 mL) — our first recommendation for thin, crepey under-eye skin.
  • EXO EYE ($155) — the exosome and NAD+ alternative.
  • Pavise 4-Step Essential Kit — Powerwash, Bioadaptive Stress Repair, Dynamic Age Defense SPF 30 and Precision Eye Lift, $57 less than buying them separately. Daily sunscreen matters as much as the eye product for pigment.

Under-Eye Hollows: Common Questions

Can you fix under-eye hollows without filler?

You can make mild hollows look less obvious by improving the skin over them, but you cannot fill a true hollow without adding volume. For a deep tear trough, filler, fat repositioning or eyelid surgery are the options that change the shape.

Is there an eye cream that actually works?

For thin, crepey, dull skin, yes, with consistent use over weeks. For bags and deep hollows, no cream will match a procedure. Individual results vary.

How long does it take for an eye cream to work?

Give it at least four to six weeks. Pavise documents photographic results at four weeks for Precision Eye Lift. Individual results vary.

What is the difference between dark circles and hollows?

Dark circles are a color: pigment, or vessels showing through thin skin. Hollows are a shape: a groove that casts a shadow. Many people have both, and they need different approaches.

What helps crepey under-eye skin?

Daily sunscreen, no rubbing, and an eye treatment aimed at density and hydration. In-office options such as laser or microneedling can help more when a topical is not enough.

Can I use an eye cream after blepharoplasty or filler?

Usually yes, once your surgeon or injector clears it. Neither procedure improves skin quality, so a good eye treatment complements both.

Individual results vary. The products discussed are cosmetic products and are not intended to diagnose, treat, cure or prevent any disease.
