The Case at a Glance
Patient: Mother of three, Age 31 · 5′7″, 150 lbs
Desired Goal: Full B to Small C — natural, proportional, athletic
Implant: Motiva Ergonomix® Demi
Size: 230cc
Technique: Preservé™ (Tissue Preservation)
Lift Note: She came to her consultation unsure whether she would also need a breast lift. Her nipple position was good, which told me the change she was seeing after three pregnancies was volume loss rather than descent. Volume is something an implant can restore on its own, so no lift was needed.
"Will I need a lift, or can implants alone fix this?" It is one of the most common questions I hear from mothers, and it is a fair one. A lift adds scars and a longer operation, and nobody wants either if they can be avoided.
This patient asked me that same question. She is 31, a mother of three, 5'7" and 150 pounds, with an active, athletic build. She had always had relatively small breasts, and after three pregnancies and breastfeeding she had lost the upper fullness she once had. She was not trying to become dramatically larger. Her goal was a full B to small C cup that looked natural on her frame. What she could not tell from the mirror was whether she needed a lift to get there.
Do You Need a Breast Lift or Just Implants?
Pregnancy and breastfeeding can change the breast in two distinct ways, and the two are easy to confuse. The first is volume loss. The breast deflates, the upper pole flattens, and the breast looks emptier than it used to. The second is descent. The skin stretches, and the breast, including the nipple, sits lower on the chest.
An implant restores volume. It does not reposition a nipple that has dropped. So the deciding question in my exam is not how much the breast has changed. It is where the nipple sits relative to the breast crease, the fold where the breast meets the chest wall.
If the nipple is still well positioned and the main change is lost volume, implants alone can often restore the shape. If the nipple has dropped significantly, a breast lift is likely part of the plan. That assessment requires an in-person exam. For this patient, it was clear: her nipple position was good. What she read as sagging was largely deflation.
Why a Demi Profile Fit Her Goal
Once we knew implants alone would work, the next decision was the implant itself. I start with the width of the breast and chest, not the cc number. Once I know the footprint that fits a patient's body, I use projection and volume to create the fullness she wants.
Motiva Ergonomix® comes in different projections. When a patient wants more fullness and a rounder shape, I typically move from a Demi to a Full. In our 275cc petite case, a 5'2" patient needed more upper fullness from a narrow chest, so the Full profile made sense. This patient's goal was the opposite: subtle. At 5'7" with a broader, athletic chest, she had the width to accommodate the implant, and the Demi's softer, lower projection kept her result natural and proportional to her frame.
At 230cc, the implant restored the volume she had lost without changing her proportions. For a sense of how similar volumes look on different bodies, compare our 150cc ballerina case and our 205cc post-pregnancy case. You can also review our guide to breast implant options in Austin.
How Motiva Preservé™ Supports a Natural Result
Her augmentation was performed with Motiva Preservé™, a less-invasive, tissue-preserving approach. The implant is placed in the pre-pectoral space, above the pectoralis muscle, without cutting or disrupting it. I create a precise pocket that supports the implant closely, rather than an oversized one, which helps keep a modest implant exactly where it was placed.
For an active patient, leaving the muscle intact also avoids the animation deformity that can occur when an implant sits beneath a contracting chest muscle. Recovery tends to be simpler as well. I ask patients to dedicate the first three to four days to rest, and most return to normal daily activities around day four, with the gym typically resuming between two and four weeks. Our Motiva Preservé recovery timeline covers this in more detail.

In the front view, her nipple position changes very little from before to after. It did not need to. The difference is in the volume above and around it: the upper pole is full again, the shape is soft and rounded, and the result sits in proportion to her shoulders and frame.
What This Case Means If You Are Weighing a Lift
- Deflation is not the same as sagging. Lost upper fullness after pregnancy is volume loss, and implants are designed to restore it.
- Nipple position is the deciding factor. If it is still well positioned, implants alone are often enough. If it has dropped significantly, a lift is likely part of the plan.
- A subtle goal calls for a softer profile. On a taller, athletic frame, a Demi profile keeps the result natural and proportional.
- The cc number is the last decision, not the first. 230cc worked here because it fit her chest and her goal, not because of a target cup size.
- Only an in-person exam can answer the lift question. Photos and mirrors cannot measure nipple position against the breast crease.
"The goal isn't to choose a number of cc's to make you a C cup. It's to choose an implant that fits your breast beautifully and creates the fullness and proportion you're looking for." — Dr. Rocco Piazza
Breast Lift vs. Implants FAQ
How do I know if I need a breast lift or just implants?
It depends on where your nipple sits relative to the breast crease. If it is still well positioned and the main change is lost volume, implants alone may restore your shape. If it has dropped significantly, a lift is likely part of the plan. This has to be assessed in person.
Can implants lift sagging breasts?
Implants restore upper fullness, which can make the breast look perkier, but they cannot reposition a nipple that has descended. When the issue is true sagging rather than volume loss, a lift is what corrects it.
What does 230cc look like on a 5'7" frame?
On this patient, 230cc Motiva Ergonomix Demi implants created a full B to small C cup that stays in proportion to her shoulders and athletic build. How any size looks depends on your chest width and existing breast tissue, which is why I size from your anatomy rather than a cup goal.
What is the difference between Motiva Ergonomix Demi and Full?
The difference is projection, meaning how far the implant extends forward from the chest. Demi sits softer and lower for a subtle result. When a patient wants more fullness and a rounder shape, I typically move to a Full. The right choice depends on your breast width, tissue and goals.
Is Motiva Preservé a good option after pregnancy?
For many mothers who want their natural volume back, yes. Preservé is suitable for a broad range of patients, and it is designed to preserve as much of your natural anatomy as possible. Read more about who is a good candidate for Motiva Preservé.
