Hairline Lowering

The procedure helps people from Austin, San Antonio, and other nearby communities correct an overly long or high forehead while preserving the natural flow of the hairline. Traditionally, an aesthetically pleasing forehead should be around one-third of the length of the face. Can You Benefit From Hairline Lowering?

Both men and women with a high hairline or large forehead can be good candidates for hairline lowering surgery. This procedure is ideal for patients who find it hard to conceal their high forehead with hairstyles. Forehead reduction is also frequently included in facial feminization procedures. Men can also have very high hairlines; however, it is commonly the result of male pattern hair loss. These individuals will be better candidates for hair replacement surgery.

Normally, the hairline starts about 5 to 6.5 centimeters above the eyebrows; however, the overall shape of the face is an important factor in determining the ideal hairline height. Nevertheless, patients who have hairlines higher than this range and feel that their appearance is negatively impacted will usually be happy with the results of hairline lowering/forehead reduction surgery.

Your Procedure

Dr. Antunes performs hairline lowering through a hairline incision—also called a trichophytic incision. This incision is meticulously designed along the hairline in an irregular wavy fashion to follow the contour of the patient’s natural hairline. The incision is placed just behind the first few hairs of the hairline; hair eventually grows back within the scar, hiding the appearance of the scar. The scalp is then released, relaxed, and carefully advanced into the forehead. The excess forehead skin is then removed in a way to avoid any tension along the incision lines.

Can Hairline Lowering Be Combined With a Brow Lift?

These procedures can absolutely be—and often are—performed in combination. The incision is placed in the same location—along the hairline. From there, the hairline is advanced forward, and the eyebrows are lifted upward.

Can Hair Transplant Surgery Lower the Hairline?

In certain cases, hair transplant surgery can also be used to reduce the size of the forehead. This can be a good option for those who have male or female pattern hair loss—receding and/or thinning of the hairline. A hair transplant can correct patchy or thin hair in the hairline or frontal scalp by adding fullness in the form of hair grafts. Hairline lowering surgery provides a more dramatic result than hair transplants because it provides normal thickness and texture more immediately (as opposed to waiting for the transplanted hairs to grow back).

Recovery & Results

The majority of the healing after hairline lowering takes 2 to 3 weeks. This timeframe will vary from person to person and also if additional procedures (such as a brow lift) were performed at the same time. Temporary numbness along the forehead and scalp may also occur, but this will normally resolve within the first few weeks after surgery.

You should avoid strenuous physical activity for 2 weeks. Most patients do not experience much pain after surgery but may notice a tight feeling along the forehead and scalp and some discomfort from the swelling. Depending on your type of work and the extent of your procedure, you may return to your regular activities as early as 10 days after surgery.

Several follow-up appointments scheduled during the first 2 weeks after the surgery give Dr. Antunes the ability to closely monitor your progress. Most patients have many questions during the initial weeks of recovery, and Dr. Antunes believes meeting with him frequently alleviates any anxiety you may have about the recovery process. His primary objective is to provide a safe, comfortable surgical experience that produces the results you desire and makes you happy. Patients have direct access to him at all times, if necessary.

You can expect to be 75% healed by 4 to 6 weeks after the surgery and 100% healed after 6 months.

Hairline Lowering/Forehead Reduction at a Glance

Goal: To restore balance to the face by reducing the size of the forehead in proportion to the other portions of the face, creating a more attractive appearance overall.

Procedure: Involves an incision made along the hairline through which the hair-bearing scalp is advanced into the forehead and a strip of forehead skin is removed.

Anesthesia: General anesthesia or deep sedation with local anesthesia.

Length of procedure: 1 to 1.5 hours

Aftercare: After surgery, the forehead may be taped or loosely wrapped to minimize swelling and bruising. Most patients go home comfortably on the same day and are cared for by family or friends. Typically, patients do not experience much pain after surgery but rather describe a feeling of tightness and a headache in the temple region. You will be given specific instructions that include how to care for the surgical site to aid healing, specific concerns to look for at the surgical site or in your general health, and when to follow up with Dr. Antunes.

Recovery: Ranges from 1 to 2 weeks on average. Some mild bruising, swelling, and tightness of the forehead and temples can be expected during the first few weeks. It is important to keep your head elevated above your heart. Some patients may experience headaches that last for 1 to 2 days. Dr. Antunes will see you several times during the first 2 weeks after surgery, and you will have direct access to him at all times. Because many questions arise during the first 2 weeks following any surgery, Dr. Antunes believes setting up a series of appointments for that time period alleviates anxiety a patient may have about the recovery process. His core objective is to provide a safe, comfortable surgical experience that produces your desired result.
